Norepinephrine

Indications

  • Septic shock (first-line vasopressor)
  • Distributive shock
  • Neurogenic shock

Adult dosing

  • drip: dose: 0.1; maxDose: 3; unit: mcg/kg/min; route: IV; weightBased: true; titrationTarget: Titrate to MAP ≥65

Pediatric dosing

  • drip: dose: 0.1; maxDose: 2; unit: mcg/kg/min; route: IV; weightBased: true; titrationTarget: Titrate to MAP ≥65

Contraindications

  • Hypovolemia (relative — resuscitate first)

Clinical pearls

  • First-line pressor per Surviving Sepsis Campaign for septic shock
  • Can be given peripherally for short duration if central access unavailable
  • Titrate to MAP >= 65 mmHg
  • Add vasopressin 0.03 units/min if requiring >0.25 mcg/kg/min of NE
