Clinical pearls
- ABC/2 slightly overestimates volume for irregular hemorrhages but is fast and validated
- Cerebellar hemorrhage is surgical at much lower volumes than supratentorial (≥3 cm often requires surgery)
- IVH (intraventricular) extension worsens prognosis significantly — note if blood is in the ventricles
- Hematoma volume is a major component of the ICH Score for 30-day mortality prediction
