Clinical pearls
- MoCA is more sensitive than MMSE for MCI (sensitivity ~90% vs ~18%)
- Clock drawing alone is quick but misses many MCI cases
- Education adjustment (+1 for ≤12 years) improves specificity in lower-education populations
- Serial MoCA scores tracking change over time are more useful than a single measurement
